Medicaid Targeted Case Management (MTCM) services involve coordinated care activities that help eligible individuals access essential medical, social, psychological, educational, financial, and other support services necessary to meet their health-related social needs. These services are designed to promote each member’s highest possible level of independent functioning within the community.
MTCM is a structured, time-limited intervention focused on empowering individuals to sustain their optimal level of independence once services are concluded. The process is collaborative, with the Case Manager working closely with the member and their support system such as family members or caregivers throughout each phase, including assessment, planning, problem-solving, and connecting to appropriate resources.
